Request a sick (fit) note

If you have been off work for 7 days or less, you cannot get a GP fit note. Your employer is legally obliged to accept a self-certify fit note, which is available via the government's website.

If you are off for more than 7 days, you will need to book a same-day appointment via the usual method, as we cannot issue a fit note without consulting you, either over the phone or face-to-face. Note that we cannot pre-date or back-date fit notes.
