Changing your contact details

To update details like your name, address, phone numbers or email address:

Change your address

If you have moved to a new address, you will need to be in our catchment area to stay registered with our surgery.

Changing your phone number or email address

You will need to show us identification before we can change your phone number or email address.

Changing your date of birth or name

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.

If you have a preferred name that differs from your legal name, we can add this to your record.  

Changing your legal gender 

Please provide us with a gender recognition certificate. We are a trans*-inclusive practice, and we will listen to you to ensure that we address you in a way that you are comfortable with. Even if you do not legally change your gender, you can contact us to inform us of your new preferred name and pronouns, which we are happy to use in informal settings.
